Patient’s Rights and Responsibilities

The physicians and staff at Silver Lake Medical Center respect the dignity, comfort and privacy of each patient and will treat you with compassion, courtesy and respect at all times.

As a patient at Silver Lake Medical Center, you have the right to:

  • Receive considerate, respectful care in a safe setting, free from abuse, harassment or neglect.
  • Receive medically appropriate and necessary care based solely on your clinical needs.
  • Make informed decisions about your medical treatment based on complete and understandable information from your physician.

HIPAA Notice of Privacy Practices

Silver Lake Medical Center is committed to maintaining confidentiality and privacy of patient and other hospital information in accordance with legal and ethical standards, and as required by the Health Insurance Portability and Accountability Act of 1996 {HIPAA]. Information about a patient may be disclosed only as authorized by the patient, supported by a legitimate clinical or business purpose, or as otherwise permitted by law.

Silver Lake Medical Center wishes to promote open communication regarding your hospital experience. We encourage you to call the respective Department Director at (213) 989-6117 for the Downtown campus, or 626-288-1160, ext. 811 for the Ingleside Campus. You may also call our Quality Management Department at (213) 989-6100, extension 6759 to voice complaints, concerns, ask questions, or to give a compliment.

If you have an issue that is still not resolved, or you do not wish to use Silver Lake Medical Center's usual complaint process, you have the right to contact any of the following agencies:
